Care Management for Adults with Disabilities

An individual assessment will be conducted to determine the type and amount of care needed for the disabled adult. Based on that assessment, a care program willbe developed to assist the disabled adult perform the required activities.

 
Relocation Services

The relocation team helps clients overcome the challenges of moving which can be disorienting and overwhelming to the client and their care providers. The program helps facilitate a move that is minimally disruptive for the client. The program also assists individuals with reorganizing and downsizing within their homes in order to age in place.
